Application Forms for Health Care Providers interested in joining the ASCH
Directions when using these forms:
Use your left mouse to click on the appropriate areas (fields) that can be filled out or you can also use the Tab button to move between fields, Shift + Tab keys to reverse direction. When you have completed all your details, print out the form and then sign it before returning it to the ASCH.
